Vision problem after age 45

If you are over 45 years of age, you've probably noticed changes in your vision. Difficulty seeing clearly for reading and close work is among the most common problems adults develop between ages 45 to 60. However, this is also the time when other changes in your eyes can start to affect your work and enjoyment of life.

Beginning in the early to mid-forties, most adults may start to experience problems with their ability to see clearly at close distances, especially for reading and computer tasks. This normal aging change in the eye's focusing ability, called presbyopia, will continue to progress over time.


What is Presblyopia

Presbyopia is the loss of the ability of the eye to change focus, such as from far to near, because of the age-related thickening and hardening the eye crystalline lens. Presbyopia is caused by an age-related process. This differs from astigmatism, nearsightedness and farsightedness, which are related to the shape of the eyeball and are caused by genetic and environmental factors. The problem usually becomes noticeable around age 45.

You can't escape presbyopia, even if you've never had a vision problem before. Even people who are nearsighted will notice that their near vision blurs when they wear their usual eyeglasses or contact lenses to correct distance vision.


Presbyopia Symptoms and Signs

Initially, you may find you need to hold reading materials farther away to see them clearly. Print in the newspaper or on a restaurant menu may appear blurred, especially under dim lighting. If you already wear prescription glasses or contact lenses to see clearly in the distance, the near vision changes caused by presbyopia will bring about the need to use bifocal or reading glasses. If you are nearsighted, you may have discovered that you now need to remove you glasses to see better up close.

Presbyopia will limit or make some activities such as sports, golf, diving, reading, looking at mobile phone, computer use or looking on the price tag when shopping more difficult due to dependency on eye glasses or bifocals. This condition will progress over time until ages reach mid 50 to 60.


Refractive Lens Exchange Procedure

As we age, the lens inside our eye loses its ability to focus both near and far. Most vision correction procedures attempt to change the focusing power of the cornea. A refractive lens exchange (RLE), on the other hand, corrects nearsightedness or farsightedness by replacing the eye's natural lens, which has the wrong power, with an artificial intra-ocular lens (IOL) implant that has the correct power for the eye.

With advanced technology of multifocal IOLs, RLE will not only simply correct refractive error but also enhanced range of vision from near through far. In the other words the procedure will correct both refractive error and presbyopia in the same time.

How Does RLE Work?

In hyperopic (farsighted) or myopic (nearsighted) patients, the light coming into the eye is not focused properly on the retina. This poor focus may be due to the eye’s crystalline lens being either too weak or too strong to help focus light appropriately. By removing the eye’s crystalline lens and replacing it with an artificial lens, the light can be better focused on the retina, reducing the person’s dependence on glasses and contact lenses to see clearly. The artificial lens that is placed in the eye, called an intraocular lens (IOLs), can be chosen depending on the refractive needs of the patient’s eye.

Operative schedule in “2-days surgery” program & Time needed in Phuket.

The operation was performed on out-patient basis. Patients didn’t need to stay in hospital. The surgery will schedule as “2-days surgery” by performed first day on first eye and other eye on next day. After surgery patient needed to stay in Thailand at least 7-10 days for follow up appointment. (Most required totally about 2 weeks in phuket)


Painless surgery

A refractive lens exchange is a simple outpatient procedure. Patients lay down in surgical bed. Attentive assistants ensure that patients are very comfortable as they drape them with sterile sheets in preparation for surgery. Patients are not put to sleep, but they can be sedated with oral or intravenous medications, if necessary. Only the selected eye is "frozen" for this painless surgery. In most cases, topical drops are used to anaesthetize the eye, so no injection is necessary.


What to expect

Patients who had RLE will have chance about 90% to achieve independence from eye glass both near and far vision or decrease dependence on eye glasses to barely minimal. Most patients can perform any daily activities without glasses in 90% of what they do and this include activities that needed both far vision and near vision such as reading, computer use, play card or sports.

Visual recovery from a refractive lens exchange, in most cases, is fast and dramatic. A few minutes after uncover eye, patients are usually able to see well enough to engage in regular activities with increased visual freedom.

Side effects are minimal. Most patients experience some temporary blurring for a few days from tear film variations or some fluid in the cornea. Patients can also expect increased light sensitivity for a few weeks, so sunglasses need to be worn more often than usual during this time. Stable vision will reach after 4-6 weeks and tend to stable throughout their life, anyway, if the stable visual result is not ideal, an additional enhanced laser surgery to improve the focus is also possible after 4-5 months.
